Most children are born with feet which, with normal gorwth and development, become both structually stable and functionally efficient.
If developemental problems occur which affect this structural stability, the function may well be impaired. Previous research has shown that permanent correction is due to development but there is also evidence to suggest that persistent abnormal function results in pathological structural changes. The longer structural and functional abnormalities go uncorrected, the more extensive are the adaptive changes in bones, joints and soft tissue.
During normal gait, the sub-talar joint moves from supinated at heel strike towards pronated at mid-stance, and then back towards supinated at toe-off. The range of these motinos in the sub-talar joint determines the subsquent attitude and behaviour of the foot under weight bearing. Excessive pronation or supination can be influenced by intrinsic factors e.g. rearfoot / forefoot varus, and extrinsic factors, e.g. genu valgum / varum, as well as developmental delay due to cerebral palsy for example.
